Group Rule: Separate Mental Health vs Primary Care Dx

The group rule "Separate Mental Health vs Primary Care Dx" will allow for diagnosis codes to be assigned to Mental Health (MH), Primary Care (PC) or Both. When the rule is enabled (Security Administrators contact ICANotes Support to enable), an automatic backfill will occur where, upon entry into the diagnosis work areas of a note, existing diagnoses will have MH vs PC automatically populated based on the following:

If the ICD-10 code begins with F*, then the code will be assigned to "MH" (Mental Health)

*Excluding substance abuse diagnosis codes F10-F19, which will be classified as "Both"

If the ICD-10 code does not begin with F, then the code will be assigned to "Both"

Before the group rule is enabled, diagnosis codes are not specified as MH, PC, or Both:

After the group rule is enabled and the work areas are accessed, automatic backfill occurs:

MH vs PC can also be set when adding a new diagnosis code from a note. Click into the field and make a selection:

For billing purposes, individual users/providers can be set in the Security Center to:

Provider Type = Mental Health

Provider Type = Primary Care

Security Administrators will click into the dropdown field to set the Provider Type:

When a note/claim is prepped for electronic or paper billing with this group rule enabled, and based on the note owner's Provider Type, the diagnosis code(s) within the note will be excluded as follows:

If Provider Type = Mental Health, then All PC Dx are Excluded
If Provider Type = Primary Care, then ALL MH Dx are Excluded

If no Provider Type is set, then all codes go to the claim

For example, this sample patient has five (5) diagnoses marked as a mix of MH, Both, and PC:

After prepping the claim (Primary with Secondary) in the Billing/Productivity Report, we see on the paper CMS 1500 that only the diagnoses marked MH or Both are included due to the note owner's Provider Type being set to Mental Health, excluding the PC diagnosis code.

Likewise the ANSI for the electronic claim includes only the appropriate diagnosis codes:

For this claim, the Primary Care ("PC") designated diagnosis codes are excluded from billing.
