New Allergy/Adverse Drug Reaction Entry Workflow

  This Knowledge Base article will demonstrate the updated workflow for entering allergies and adverse drug reactions to a client's chart. This workflow was updated as part a July/August 2020 release. Each note type where the workflow applies will be contained within it's own sub-section below for easy viewing.      Complete Assessment (of Any Discipline)   Within the Complete Assessment, navigate to the Medical History tab and locate the combined ADRs and Allergies button.      If no known Adverse Drug Reactions or Allergies are present for the client, indicate so using No known ADRs or Allergies. Otherwise, proceed to List ADRs and Allergies.     The Adverse Drug Reaction (ADR) & Allergy List window has been updated to reflect entry points for both types. Notice color coding assigned to each type: ADR (yellow) and Allergy (blue). Each step is numbered and highlighted for your convenience.      1) Click New to start a new entry, then use the radio buttons to specify whether you are entering an ADR or Allergy.     2) Click into the ADR/Allergy To dropdown and make a selection from the list to populate it. If the reaction/allergy needed is not within the list, click into the yellow field to free-text the appropriate item.      3) Indicate a Reaction Date by clicking into the calendar field making an appropriate selection. If the reaction date is unknown, indicate so using the Unknown Date checkbox.      To the right, update the Status to indicate Active or Inactive, the No Known ADRs or Allergies checkbox if needed, and Reason for Status Change (if applicable - this field will mainly be used for subsequent updates to the ADR/Allergy).     4) Next, indicate the nature of the reaction by first selecting a system in the first (left) column, then reaction in the second column. Select as many as necessary; each reaction type will populate into the field on the right. Below, indicate a Reach Severity and RxNorm Code. If desired, you may Create New Reaction Button to indicate a custom reaction.      5) Click to Save the ADR/Allergy, or Cancel to undo your work.      Upon saving, the ADR/Allergy will be present within the ADR/Allergy To field on the left-side of the screen. Follow the same workflow (click New to start again) as noted above to continue making additions as needed, until all Adverse Drug Reactions and/or Allergies are present.    To make changes to an entry before returning to the note work areas, click on the appropriate ADR or Allergy to open its entry work areas, make the necessary changes, then click to Save again.      6) When finished entering all ADRs/Allergies, click Back to return to the note work areas.     The note will updated to reflect the entered information.      Adverse Drug Reactions and Allergies will display in the Chart Face:     As well as within the updated ADR/Allergy Log (with yellow and blue color labelling to indicate ADRs and Allergies, respectively):         Prescriber Progress Note   Within the Prescriber Progress Note, navigate to Drug Reactions in PN, Part 1.      If no known Adverse Drug Reactions or Allergies are present for the client, indicate so using None button. Otherwise, proceed to Drug Reactions.     The Adverse Drug Reaction (ADR) & Allergy List window has been updated to reflect entry points for both types.
