How to Add an Allergy

Overview 

This Knowledge Base article explains how to add allergies for clients. You can enter details such as onset date, end date, allergy type, reaction type, and severity. Once the onset date has passed, the allergy becomes inactive. Additionally, you have the option to archive the allergy. When you print the allergy report, it displays a list of allergies categorized by type.

 

Adding an Allergy 

You can add an allergy from the progress note created for the client.

 

Navigate to the client's chart and create a progress note. Within the PN, Part 2 tab on the Progress Note, locate the Medication box as highlighted in the screen below:

 

Note: The Medication box is visible only to users with access to the eRx feature.

 

 

 

In the Medication box, you can view prescribed medications and allergies recorded for the client. To add a new allergy, click the Allergies / ADR button.

 

 

The Allergies / ADR tab opens and displays a list of allergies recorded for the client. When the client is not allergic to any food or drug, you can select the No Known Allergies option.

 

 

 

Search for a new allergy and select it from the list as shown below.

 

 

 

 

On the Allergy screen, select the onset date and end date for the allergy. Record other details such as allergy type, reaction, and severity in the screen below.

 

Type: The allergy type could be: Food intolerance, Food allergy, Drug allergy, Drug intolerance, and so forth.

Reaction: The reaction type could be: Skin rash, Hives, Respiratory Distress, Diarrhea, Dizziness, and so forth.

Severity: The severity could be: Fatal, Severe, Moderate, and so forth.

 

 

 

Once allergy details are added, click Save

 

The recorded allergy appears in the list as shown below. Click the pencil icon from the extreme right side to make any modifications to the allergy details.

 

 

 

The allergy details can be accessed from the PN, Part 2 tab on the Progress Note, as depicted below:

 

 

 

Archiving an Allergy 

On the Allergies / ADR tab, click the Archive icon from the extreme right side. A confirmation message appears to archive an allergy as shown below:

 

 

 

 

 

To access the archived allergies, click the  icon and select the Archive option from the list.

 

 

 

This shows only archived allergies as shown below:

 

 

Accessing Inactive Allergies 

On the Allergies / ADR tab, click the  icon at the top and select "Inactive" from the list.

 

Inactive allergies are those for which the onset date has already passed.

 

 

This shows only inactive allergies as shown below:

 

 

 

 

Printing the Allergy Report 

On the Allergies / ADR tab, click the printer iconfrom the top as shown below. 

 

 

This opens the Patinet Allergy Report displaying client's active and archived allergies.
