Creating a BIRP Note

This knowledge base article covers the workflow of creating a BIRP therapy note in ICANotes.    From the patient's Chart Face, start by creating a new Therapist progress note.   Screenshot for Creating a BIRP Note   Within the work areas of the Therapist note, you'll create a series of custom buttons so each part of the BIRP can be completed. From the bottom of the Custom Buttons work area, cick to Create New PN (Non-Rx) Button.   Screenshot for Creating a BIRP Note   In the Add a Custom Button pop-up window, you'll give each button a name (1), provide the text to include within the note (2), set highlighting and sharing options (3), and then Continue (4) back to the main work areas of your note, saving the custom button.   Screenshot for Creating a BIRP Note   We recommend setting up a series of buttons to constitute the BIRP note, as follows:   BIRP: 1. BEHAVIOR or 2. INTERVENTION 3a. RESPONSE: (good) 3b. RESPONSE: (poor) 3c. RESPONSE (none) 4. PLAN:   Use the first variation within the custom button to build out each sub-header (Behavior, Intervention, Response, Plan), then the work areas to complete each section of the BIRP note. Once each button is created, you can change their position within the custom button listing by clicking Edit next to any button to return to the custom button pop-up.   Screenshot for Creating a BIRP Note   In the pop-up, use the arrow buttons located under the title of each button to adjust the order. This will make it so that you can place your BIRP buttons in their correct order.   Screenshot for Creating a BIRP Note   Here, the initial BIRP custom button is in the 10th position. Click the arrow arrow on the left in the green highlighted area to move the button to the top, or use the second arrow button on the right to move incrementally up space-by-space until the buttons are in the desired order:   Screenshot for Creating a BIRP Note   Click Finished once you're satisfied with your changes. You'll see the new display order of your custom buttons within the work area of the note.   Screenshot for Creating a BIRP Note   Once the buttons area created, they will be available to you use within the note type for all subsequent patients. For a more comprehensive look at creating and using custom buttons, please see this knowledge base guide.    Start building your note by clicking on the first button: BEHAVIOR. Once clicked to establish the header, navigate to the MSE/Appearance, or you may write your own narrative to denote behavior.   Screenshot for Creating a BIRP Note   Once the MSE is completed, click Back to return to the main work areas of your note. If you prefer, you can remove the text "MENTAL STATUS EXAM" header that is automatically applied when using the section. This can be done simply by backspacing or deleting the text.   Screenshot for Creating a BIRP Note   For INTERVENTION either click on the custom button or go to the Therapy Intervention shrub.   Screenshot for Creating a BIRP Note   As with the preceding step, complete Therapy Intervention, then click Back to return to the work areas.   Screenshot for Creating a BIRP Note   Next you'll use one of the custom buttons you created to note the patient's RESPONSE (good, poor, or none). The variation text entered in the custom button will appear within the work areas of the note, expanding on your content. However, you are able to free-text in your own content by simply typing into the yellow text work area, if the programmed custom button doesn't suit a particular client's needs.   Screenshot for Creating a BIRP Note   Lastly, for PLAN you can use free-text, other custom buttons,  or the Link to Treatment Plan option if the patient has an established treatment plan.    Screenshot for Creating a BIRP Note   In the Link to Treatment Plan window, select the name of the Treatment Plan for inclusion, then select a Problem.    Screenshot for Creating a BIRP Note   Upon doing so, you'll be presented with the option to Link Intervention(s) from the Treatment Plan into the BIRP note. With the desired Interventions selected (the green check mark means the intervention is selected), click to Link Intervention(s). If you do not wish to link any interventions,  you can instead click Back to return to the previous screen.    Screenshot for Creating a BIRP Note   The Treatment Plan will now be present within the work areas of your BIRP note. Additionally/alternatively,  you could also/instead utilize buttons in the Progress in meeting goals and resolving problem(s): area to provide a brief update to the plan. When finished, click Back to return to the main work areas of your note.    Screenshot for Creating a BIRP Note   Finish the BIRP note by setting a diagnosis code (if needed), return visit, length of session and service code.
