Consolidated Clinical Document Architecture (CCDA) is a base standard which provides a common architecture, coding, semantic framework, and markup language for the creation of electronic clinical documents. CCDA is available to patients on the Patient Portal and can also be exported as a report from the Upload Site.
This is turned on when the Patient Portal is turned on for a group; however, this can also be turned off if the group does not want the patient to have access to the CCDA. The CCDA is updated every time a note is compiled. Note: Generating CCDA is a requirement for Meaningful Use.
The following is included on a CCDA:

This is an example of a CCDA with all fields filled out (as seen from the Patient Portal):
Customers reporting on MIPS or Meaningful Use now have the ability to export CCDA’s from the Upload site. Because this is date-driven, it will only export an encounter (note)-based CCDA’s and not the full CCDA. Customers not participating in Meaningful Use will have the option to export CCD using a date range. However, no data will be found.
Begin by logging into the Upload site with your ICANotes credentials, at https://upload.icanotes.com
Click on 'CCD Exports'. Here you will have the ability to create a new CCD export, or view upcoming (scheduled) and recurring exports.
To create a new export, click 'Create New Export'. You can export immediately, or schedule for a future date and time. You can also schedule an export to run at a future time based on a recurring frequency.
You will need to add a Description and select the Frequency of your export.
If you choose to export CCDA at a later time, the Schedule field will expand to give you options to set a date and time.
a. Set the desired date by clicking on the Calendar Icon to populate a Calendar. Use the arrows on the Calendar to move month to month. Click on the desired date. Once set, the date will populate in the corresponding field.
b. Click on the Clock Icon to select the time you would like the report to be run. Use your mouse or keyboard arrows to scroll through the hour, minute and AM/PM options. Once set, the date will populate in the corresponding field.
To schedule a recurring export, the Schedule field will expand accordingly. Scheduled CCD exports can recur every week or every month on a specific date and time. Use the drop down arrows to select values for each of the fields 'Recurs Every', Recurs On' and 'Recurs At'.
After giving your export a name and setting the frequency, set the date range for your export data
a. Click on the first calendar icon to populate a mini calendar to select the date you would like your export to begin.
b. Click the second calendar icon to so set the stop date using a mini calendar, as in the step previous.
To create an export for specific patients, follow the steps below.
1 – Click the radio button to select 'Selected Patients'.
2 – Enter a patient’s name and click 'Search'.
3 – To add a patient to the list, click the '+' button
4 – A list of patients will start to build at the right. To remove a patient, click on the trash can icon.
Once you have set up your report to your specifications, click Submit.
You’ll see your export in the Upcoming or Recurring Exports list
Once a CCD export runs it will appear in the Exports dropdown for download (.zip)
Locate the downloaded .zip file on your computer (check the Downloads folder)
