Portal-enabled patient signature is part of the premium patient portal service provided by ICANotes. For more information about the premium patient portal service, please contact Sales.

These are instructions on how to upload forms for your patients to sign through the patient portal and how to find the forms in ICANotes once signed. 1. As a Patient Portal Administrator, go to the ICANotes upload site. Enter your User name and Password. Click the Sign In button.
2. Click the Group Forms button.
3. Under "Upload a Form" choose the category that you would like to use from the dropdown menu.
4. Type in the Form Name.
5. Click the 'Chose File' button and locate the form on your desktop. Select the form and click the 'open' button.
7. Click the 'Upload' button.
You will see a notification at the bottom left indicating that the form was successfully uploaded.
8. The form will now be under Consents and Legal Forms under the Forms (middle column)
This form is now available on the patient portal for signature as shown below.
9. To find the signed document, go to the Document tab from the Chart face.
10. The documents will be listed with the most recent at the top.
11. Click the 'View this Document' by the patient form.
12. The last page will show the patient's signature.
13. The form can also be found on the Upload site by searching for the patient and looking under the Intake documents.
14. Click the download icon to open the document.
15. Scroll down to the bottom to see the patient's signature.
