We have introduced a new alert notification in situations where a locked MAR record prevents a medication order within a note from taking place. Further details are provided below.
A MAR order is "locked" when a user is making a type of change to it. This happens on the FileMaker side and is quite similar to situations where someone is modifying patient records in Demographics while another user/clinician is trying to compile a note for that patient. The note taker will receive an error message notifying them that another user is locking the record by making a change. The same principle applies here.
If Person A is modifying the medication record from the MAR, such as adding a note or changing the administration time, this will cause the record to "lock" within the program. If Person B is attempting to make a change to that same medication within note work areas while the record is locked, Person B will receive an error message upon compiling their note informing them that their change to the medication order was not sent to the MAR because the medication was locked at the time. The medication name and order will be identified within the error for clarity. The error message will recommend Person B (the note taker) check the MAR for the original medication order and make a note alerting others to not administer. They are also encouraged to notify their charge nurse - or someone with the "Lock MAR Record" privilege enabled - to lock the medication order from the MAR so that it can no longer be administered (if the note taker does not have that privilege enabled themselves).
Example #1: Person A is adding a note to a medication order on the MAR. When their cursor is active in the field (seen below), the change (text) being made has yet to be committed, locking the record. Once the cursor leaves the field and the text is committed, the record is no longer locked.
Example #2: While Person A is active within the field, Person B is attempting to make a change to the same medication order within a progress note.
Example #3: Person B receives an error message upon compiling their note, notifying them that the specific order was not updated due to the record being locked.
The error message does not prevent compilation, and whatever changes Person B was trying to make in the identified order will appear on the finished note and in the medication log. However, it will not show on the MAR because the record was locked at the time. Please note, however, that if in the same note you are creating a NEW medication order, that order will not be impacted by the locked record. The error message is not preventative; if it appears, the error has already occurred and the MAR order should subsequently be noted/locked to prevent unnecessary administration to the patient.
Details of the error will simultaneously be sent to ICANotes (to the Clinical Director and MAR developers), who can at that point look at the data and determine if the error arose from two users modifying the same record at the same time (where a lock occurred), or if something else caused the error that requires further troubleshooting and possible programming changes on the ICANotes side.
