How to Reconcile Medications
Overview
Medication reconciliation ensures that clients receive the correct medications at all points of care. ICANotes+ allows you to mark medications as reconciled, which will then appear on the compiled note.
Step 1: Access a Note
- Open ICANotes+.
- Navigate to a note that allows you to prescribe medications (e.g., Progress Note).
Step 2: Open the Medication List
- In the Medications section, click the Enter button.
- The Medication List tab will appear.
Step 3: Mark Medications as Reconciled
- Locate the medication(s) that need reconciliation.
- Select the Medication Reconciled checkbox.
Step 4: Compile and Preview the Note
- Click the Compile and Preview button.
- Review the compiled note to verify that the reconciliation status appears.
Step 5: Confirm Reconciliation
- Locate the section in the compiled note where it states that "medication reconciliation has been done."
- Ensure that the information is correct before finalizing the note.
