This knowledge base article will demonstrate how to enter an ICD-10 diagnosis. Diagnosis fields are present in most notes.
1. From any diagnosis fields, click the Enter button.

2. There are three ways that can be used to enter ICD10 codes:

3. Once you have entered all of the diagnoses, click the Done button.


4. Next, decide if you want to include the diagnosis in the note.
- Yes - the entire diagnoses will show including the Axis II-V.
- Omit All - no diagnoses will show in the note.
- Omit Axes II to V - only the diagnosis shown in the field will show.
5. You are able to enter Axis II - V by clicking the
button. Fill in the Diagnosis Details.
