The tab that you are currently viewing will be highlighted with a green stripe at the top of the tab, in this case, Patient Information. If you hover over another tab, that tab will be a darker color and will also have a green stripe.
If your practice will be billing electronically, using the 'Show' button to highlight fields used by electronic billing will be helpful. This button is at the bottom of the Demographics Page.
Name (First and Last), Refer To Patient As, Date of Birth, Gender, Address (Street, City, State and Zip), at least one phone number, Unique Patient ID and Assigned Providers are necessary to save demographics. These are the minimum required fields.
This is used generally for inpatient treatment. There are three yellow fields that should be addressed. Default is "No" to all answers.
This is where you can add insurance information. For additional information about entering Insurance Information see the related Knowledge Base Article.
This is where you can add contacts.
This tab is only available when a chart is first created and can be used to add additional information regarding the intake.
There are several options when the Demographic Entry is complete:
1. 'Save and go back to Chart Room' and 'Save and go back to Chart Face' at the top of the chart will allow the information entered to be saved without generating a note.
2. 'Continue to Initial or Complete Exam'. Skip Initial; create a Group Therapy Note' and Skip Initial; create a Progress Note' will all take the user to the respective note type after saving the Demographic Information.
3. You can also Cancel Chart Creation and Return to Chart Room if desired.
You can visit Demographics and the Telephone Intake Form from the Chart Face.
The Telephone Intake Tab will no longer be present in the Demographics after the first save. Updates can still be made. Click 'Continue' When updates are complete.
After the initial save, the Telephone Intake Form looks like this, and can be printed.
