When designing forms in the EHR, you can choose how they appear to your team:
Sections (step-by-step): Breaks the form into smaller parts with navigation buttons. Recommended for longer forms.
One Page (headings): Displays the entire form on a single screen with bold headings. Recommended for shorter forms.
Both layouts support the same fields, signatures, and conditional (if/then) questions.
The form is divided into smaller sections (for example, Patient Info → History → Consent).
Navigation is provided with Next and Back buttons.
Required fields must be completed before moving forward.
Scrolling is reduced, which provides a streamlined experience for longer forms.

Choose the One Page layout when you prefer to display all questions in a single view. This layout includes:

Clear headings that organize related questions.
A layout that makes the form easy to skim and quick to edit.
A design that works best for shorter forms, such as simple consents or updates.

Form length: Long forms work better with Sections. Short forms are easier with One Page.
Task duration: Quick tasks are better suited for One Page. Longer tasks are easier to manage with Sections.

Your preferred layout: Sections or One Page.
Your logo file (JPG/JPEG), if you would like it displayed on the form.
