Encounter Form

The Encounter Form is a running log of signatures collected from the client and clinician on each visit and includes optional fields such as the Medicaid Number, start time, end time, units, and service code. The report can be printed and saved as a PDF to the client's Documents tab (and Upload site). Encounter Form notes do not appear in the Chart Face and are only visible from the Documents tab and the Upload site. 

Security Administrators can contact ICANotes Support via phone (443-569-8778) or email (ticket@icanotes.com) to request enabling the Encounter Form for their account.

With the group rule enabled, you'll find the Encounter Form toward the bottom of the patient's Chart Face:

Click Encounter Form 

When you access the Encounter Form, some patient information will automatically populate for you: 

A - ID number 

B - Today's Date

C - Medicaid Number (if entered in Demographics)

The remaining fields can be free texted as needed before capturing the Client and/or Doctor signature(s):

Once a signature is collected the respective row is locked from editing. Signatures can be overwritten, and rows can be deleted. Each time the Encounter Form is accessed you will follow the same simple workflow on the next available line item.  

To delete a row, click anywhere within the row to highlight it, then click Delete Selected Signatures

You'll be prompted to confirm that you want to delete the selected signatures

Text: Please verify that you want to Delete the selected signature(s) associated with ID 1 & last signed at (date and timestamp)

Click Delete to confirm and the line item/signatures will be removed from the form

At the top of the Encounter Form you have options to Save as PDF and Print Form

Click Print Form to print a copy to your printer, or select the Save as PDF option in the print dialog box to save a PDF copy to your local device.  

Click Save as PDF to save a copy of the Encounter Form to the Documents tab of the client's chart, as well as to the Upload site. You'll receive a prompt confirming it has been saved:

To view the saved file, navigate to the Documents tab of the patient's Chart Face and look for "Encounter Form".
