Within ICANotes+, you have the capability to perform assessments and automatically generate ratings based on these assessments. These ratings can be integrated into various notes, including Progress Notes, Case Management records, Complete Evaluations, and SOAP Notes. There are two methods available for conducting assessments:
To initiate an assessment within ICANotes+, follow these steps:
Navigate to the client's chart > access the Chart Face tab and click the Forms / Assessments button, as depicted below:
From the Category dropdown menu, select Clinical Rated Assessments.
Within the Form dropdown, select the specific assessment to administer to the client. For example, select PHQ-A from the list.
Once both Category and Form are designated, click the Create button.
The form will appear for completion. Respond to each question appropriately, then click Submit.
Upon submission, a PDF will be generated as illustrated below:
Scrolling down in the PDF, you will find the automatically calculated PHQ-A score based on the provided responses.
To initiate an assessment through client portal, follow these steps:
Navigate to the client's chart and access the Communications tab.
Expand the Client Portal panel and click the Select Forms and Assessments button, as depicted below:
On the Send Forms and Assessments screen, expand the Assessments section and select the specific assessment to administer to the client. For example, select PHQ-A from the list. Select the Send Form Request Email button as depicted below.
The form will be sent to the client portal user and will appear with the In-progress status, as depicted below:
The client portal user will see a notification in the My Documents section as shown below for the assessment sent by practice.
In the My Documents section, they can see the assessment with the Pending Review status.
When they open the form, it will appear as shown below. They can respond to each question appropriately, then click Complete. Refer to the following screens.
After finishing and submitting the assessment back to the practice, the client portal user will receive a success message.
Upon completion of the form, the status will change to Completed.
When assessments are conducted and ratings are captured, either within ICANotes+ or through the client portal, these ratings can be integrated into various notes, including Progress Notes, Case Management records, Complete Evaluations, and SOAP Notes.
Example: Accessing the Progress Note to View the Rating Scale
1. Open the Progress Note.
2. From the left pane, select the Rating Scales option.
3. Choose the assessment conducted for the client (in this example, PHQ-A.)
4. The client's exact score will appear in the shrub as shown in the screenshot below.
